There are multiple computers in the library, computer lab, and classrooms available to students. As parent(s)/guardians(s) of ________________________ I/we understand that the staff/teachers cannot monitor all of the information my/our child is able to access or download at these computers.

Both my child and I/we understand that both the accessing and saving of inappropriate materials will not be tolerated. Furthermore, the accessing of such material will result in loss of privilege of having access to computers and may result in other penalties e.g., detention or suspension.

I/We also understand that it is impossible for the staff and teachers of Corpus Christi School to completely restrict access to controversial materials.

I/We will not hold the teacher, staff member, Corpus Christi School, or the Diocese of Oakland responsible or legally liable for materials distributed to or acquired from network or online services.

Please sign and return this form to Corpus Christi School by the first day of school with your completed emergency cards and the other Required Computer Forms. By doing so, you and your child indicate that you agree to abide by any restrictions regarding access to inappropriate/controversial materials as determined by you, Corpus Christi School, and/or the Diocese of Oakland, and further that you hold the School and the Diocese of Oakland blameless if your child accesses and/or saves inappropriate controversial materials.
